According to Official Guinness Records,

The longest surviving heart transplant patient is Ted Nowakowski (USA, b. 23 November 1948) who lived for 34 years and 261 days after receiving his transplant on 25 April 1983. After the transplant, Ted continued working as a teacher, and later became a principal. Ted passed away on 11 January 2018.Ted Nowakowski received a heart transplant at the University of Minnesota on 25 April, 1983.
