According to Official Guinness Records,

The most flags tattooed on the body is 366 and was achieved by Guinness Rishi (India) between July 2009 and July 2011.
Guinness Rishi already held this title which was achieved in May 2010. The latest additional 61 flags were tattooed at 'Team KDZ Tattoos Body Work' in Delhi, India, and took a total time of 3 hours 3 mins to complete.
