According to Official Guinness Records,

The most swim caps worn at once is 75 and was achieved by Hristo Terziyski (Belgium) in Kalmthout, Belgium on 4 April 2014.This record was achieved as part of a record breaking day at Gitok technical school. It was one of eleven records attempted.
