According to Official Guinness Records,

The most gender reassignment surgeries for an individual person is held by Blair Logsdon (USA) who had undertaken 167 surgeries between 1987 and 2005. As well as gender reconstructive surgery to the pelvis and on the breasts this list includes multiple facial feminisation procedures.
