The fastest long course 4 x 100 m medley by a male relay team is 3 minutes 27.28 seconds, by the USA (Aaron Peirsol, Eric Shanteau, Michael Phelps, David Walters) at the FINA World Championships in Rome, Italy, on 2 August 2009.
