According to Official Guinness Records,

The most toothpicks rotated in the mouth simultaneously is 66, achieved by Dinesh Shivnath Upadhyaya (India) in Mumbai, India, on 16 February 2018. Dinesh beat his previous record from August 2017 by two additional toothpicks.
